Our platform Tailored Talks is now being used in NHS Lanarkshire in elderly and frail care.
The Pogo team are excited to be involved in a new pilot starting today, which will support care home liaison nurses working in NHS Lanarkshire. The nurses and care home staff will now have access to Tailored Talks new content around elderly and frail care, which can be used to provide information and support to care home residents across a range of conditions.
Tailored Talks is a digital platform built for health professionals to personalise the medical information they share. It will allow care home staff to access information so that residents have a better understanding of conditions that affect them. This information can be revisited at any time, and can help to support residents, staff, family and other carers.
We look forward to working collaboratively with NHS Lanarkshire and Thistle Healthcare Residential Homes that will be involved in the pilot, and we hope that this pathway will help to facilitate an integrated health and social care system in Scotland.

"I'm really excited about Tailored Talks being launches this week in collaboration with some Lanarkshire Care Homes. The system allows clinicians to select person-centred information from a bank of slides to aid and follow up on conversations they have had. The initial focus will be on management of constipation and also ReSPECT; which is a process that creates personalised recommendations for a person's clinician care and treatment in the case of a future emergency where they can't make or express their choices. Tailored Talks will improve outcomes for residents and ensure they and their loved ones are fully involved and informed in planning care which is based on their individual needs and wishes."
